Send troubleshoot ticket to Anthony to research, Corp will follow up with customer in 24 hrs. (leave ticket open-Anthony will resolve).  Troubleshoot ticket-group: logistics


HOME/PRO:  please collect the following information from customer:

1.  Provide contact information

2.  How they ordered-through their Dr./hospital etc.

3.  Date of order
